Hidden characters are always removed from the cleaned report, and en or em dashes become plain hyphens. Safe replacements only change terms with a direct equivalent, such as "bony" to "osseous"; hedging is flagged but never rewritten.
Use one consistent scale for diagnostic certainty, and state the diagnosis outright when you are certain. Adapted from the Mass General Brigham Diagnostic Certainty Scale.

Avoid: probably, possibly, question of, suggestive of, suspicious for, may be, worrisome for, highly unlikely, cannot be excluded.

Each measure cannot drop below zero. Report structure is not scored. Grades: 90+ excellent, 75–89 good, 60–74 needs work, below 60 poor. The weights are a teaching aid, not a validated metric.
